Health and care service contract

Provision of clinical case management, assessment, and brokerage services for NHS continuing healthcare and personal health budget patient cohorts.

  • Assess and review patients funded by the ICB to ensure patient safety, effectiveness, and clinical and financial benefit
  • Negotiate and establish Personal Health Budgets where appropriate and guided, adhering to the ICB's governance and sign-off processes
  • Deliver brokerage for care packages using established provider frameworks and spot-purchase providers while supporting patient choice and personalisation
  • Assess patients to determine eligibility, agree care package costings, undertake MDT reviews, settle stakeholder disputes, escalate quality concerns, support hospital discharges, and manage relationships with healthcare trusts, Local Authorities, domiciliary homecare agencies, care homes and supported living providers
  • Meet the service's Key Performance Indicators and manage the specified caseload and referral activity to support national Quality Premium targets, NHS England/Improvement data submissions and local quality indicators
