What the contract covers

Service delivery

Provision of community-based stroke recovery support, information, emotional guidance and advocacy for survivors, families and informal carers.

Support stroke survivors in identifying and working towards their own needs, wants and goals with specialist input and guidance as required
Maintain regular contact with the stroke survivor as appropriate, including but not limited to home visits, face-to-face meetings, telephone calls, virtual meetings and sending out information
Conduct regular reviews of each stroke survivor's needs
Guide people using the service through lifestyle changes based on identified risk factors and provide stroke survivors and their families and/or informal carers with practical support and advice
Support stroke survivors to regain confidence, re-engage in community activities that are relevant to them as individuals, and aid the reduction of social isolation
Make appropriate onward referrals where additional needs are identified and with the consent of the person using the service
