Mental Health Outreach Support for Complex Needs Housing - Multidisciplinary Team

Provision of community-based mental health outreach and support services for adults experiencing homelessness and multiple complex needs.

• A free-at-the-point-of-use preventative mental health outreach service for single adults aged 18 and over experiencing homelessness and multiple disadvantage
• Individually tailored interventions centred on a case-management model delivered through assertive community outreach
• Emotional and practical support, including support with housing and debt, health and wellbeing, crisis reduction and recovery
• Advice and information, peer support groups, community hubs, telephone, email and other virtual/digital support
• Onward referrals and support to access education and careers advice, existing community-based services and agencies
• Support to build confidence, self-esteem, life skills and new social networks, using a flexible and person-centred approach that builds service-user skills and strengths
