  • Provide a single remote monitoring platform for managing patients at scale and enabling patients to securely submit health information.
  • Provide a clinical platform/dashboard for the remote monitoring service clinical team to access and provide real-time monitoring of patient data and RAG alerts 365 days per year.
  • Provide a patient-facing app/portal for patients to input agreed health data, with configurable symptom question sets and escalations.
  • Provide interoperability and a process for pertinent data to be populated into the local Connected Care Record (CHIE).
  • Enable activity reporting to be run and extracted.
