What the contract covers

Provision of befriending and social support services for socially isolated older people with long-term conditions.

  • Befriending Service will offer supportive, reliable relationships through Volunteer befrienders to individuals age 65+ living in Calderdale who would otherwise be socially isolated
  • Alleviate social isolation in older people age 65+ with long term conditions including people with physical disability, sensory impairment, early onset dementia and people moving out of hospital
  • Enhance the quality of life for the individual using the Service, their family, and carers
  • Improve health and social outcomes for people
  • Improve emotional wellbeing
  • Ensure the Service is well advertised to maximise usage
  • Take a person-centred approach with the individual building on their strengths
  • Recognising the individual's skills, abilities and things that are important to them in their life and community
