Short-term enablement domiciliary care service offering timely support for individuals who may have an eligible Care Act need through packages of personal care within their own home whilst awaiting an assessment under the Care Act (2014)
Supporting or administering medication in line with the prescriber's recommendations
Working alongside any health provisions which may wrap around the individuals core social care delivery to ensure a joined up approach to safe and effective discharge from hospital
