Service contract

Provision of community-based reablement and social care services to support adult independence and facilitate timely hospital discharge for local authorities.

Community reablement support enabling adults to maintain, retain or regain daily living skills and abilities and remain living independently in their usual place of residence or with less support.
Person-centred, time-limited, outcome-focused and strengths-based interventions that prevent, reduce or delay the need for care and support.
Reablement support for customers with physical, learning, neurological, mental-health and other long-term conditions, including double-handed care support.
Coordinated partnership working with the Council, NHS partners, Intermediate Care and Primary Care Services, hospitals and other community services.
Support facilitating timely discharge from hospital and maintaining independence at home.
