Provision of postvention bereavement support services for individuals affected by suicide, including emotional support, practical guidance, and multi-agency partnership working.

  • Deliver Postvention Bereavement Services to residents or people registered with a GP within Swindon and Wiltshire.
  • Provide person-centred postvention bereavement support, including holistic assessment, safety and support planning, one-to-one and group support, emotional support with coping strategies, practical help with police processes, funeral arrangements, inquests, media, death notifications and employer communications, and signposting/referral to other services.
  • Support wider mental-health and bereavement determinants, including trauma-focused therapy, financial and housing support, multilingual guides/resources and translation access.
  • Provide access, referral handling, risk assessment, onward support and regular supportive contact, including support through related statutory processes.
  • Gather and upload access information to MHSDS, record support conversations and demonstrate timely access and onward transfer outcomes with commissioners.
